  • Confirmation Of Gross Receipts Of $50,000 Or Less
  • A Statement That The Organization Is Going Out Of Business, if applicable

Form 990-EZ

When filing Form 990-EZ information on the organization’s EIN, gross receipts and assets must be provided. Also, all appropriate, required Schedules must be completed and attached when filing Form 990-EZ.

Form 990-PF
